The Bureau of Vital Records
maintains records births, deaths and marriages occurring in the
state of Iowa from July 1, 1880 onwards. Vital records can be
obtained from the department in person, via telephone, or by
mail. Records are restricted and are only available to certain
individuals. Fees are payable by check or money order made out
to the Iowa Department of Public Health. In-person
requests may also be paid in cash. Checks must be drawn from the
applicant's account; money orders must be in the name of the
applicant. Fees must be paid at the time of the application.

person. When requesting a certified copy of a birth record by
mail, applications must include a clear photo copy of a current
government issued ID and applicant's notarized signature.
