The Illinois Department of
Public health maintains vital records for births and deaths
occurring in the state of Illinois from 1916. As, such, they
have over 27 million records on file, including some records
which date back to the mid 1800s. If you're seeking a record for
an event that took place in Illinois, you can obtain that record
from either the Health Division of Vital Records, or from the
County Clerks and Circuit Clerks in the county where the even
occurred.
Record Type: Birth
| Dates: Since 1916 | Cost: $15.00
Illinois birth records are
restricted and can only be requested by the person named on the
certificate (18+), a parent or legal guardian, or by a legal
representative. Copy of valid photo identification is required
for processing. If the event occurred prior to 1916, the records
can be obtained from the relevant County Clerk.
Record Type: Death
| Dates: Since 1916 | Cost: $17.00
Illinois death records are
restricted and are only available to those who have a personal
or property right interest with the decedent. If you are not a
relative of the deceased person, a letter or document from the
office or agency that needs the death certificate must accompany
the request. If the event occurred prior to 1916, the records
can be obtained from the relevant County Clerk.

Division of Vital Records can
conduct a search for verification purposes only. Certified
copies of marriage records must be obtained from the County
Clerk in the county where the marriage was issued.
