The Office of Vital Records
maintains vital records for events occurring within the state of
North Dakota. Proof of identity is required with all requests
for vital records in order to prevent identity theft and fraud.
Proof of identity means submitting a legible copy of a current
driver's license or photo identification along with a vital
record request. Personal check or money order should be made
payable to ND Department of Health.
