The WI Vital Records Office
maintains vital records and issues certified copies of birth,
death, marriage and divorce records for events that occurred in
the state of Wisconsin. Records can be requested in-person, by
fax or by mail. Telephone orders aren't accepted. Fees are
established and for searches only. If a record is found for the
date/s specified, a certified copy is issued at no additional
cost. If no record is found, a refund is not issued. Records are
restricted and only available to certain individuals.

Only available to the
registrant, spouse, immediate family or those authorized. For
the $20.00 birth certificate search fee, the office will search
one five-year period in the single-year indexes OR one
